Form A is the Annual Consent and Release. We need this form on file, updated each year for each student. This form provides us with important contact and emergency information. This form should be completed for any student participating in one or more parish youth ministry activities.

Form B is the Event Specific Consent and Release. This form is an event-specific permission slip which must be completed for any event that is not held on parish property such as a hayride, or diocesan pilgrimage

Form C is the Prescription Medication Form. This only needs to be filled out if a student needs to take prescription medication during a parish or diocesan sponsored event.
